 A safe delivery of a woman in a level one hospital emergency room.

 

 Prepare for an imminent delivery, including equipment and personnel. Validating each consecutive step by your   documentation and demonstration of knowledge by your role playing
Assess the stage of labor, vital signs and the presence of amniotic fluid  and it's color
Determine pain level and provide pain relief options and support, including information about the Doula.
Continue to assess patients status and assist doctor with delivery of baby, placenta and any necessary repairs

 Assess a newborn for complications and assign the baby's Apgar scores

 

 

 

Prevent heat loss by drying the baby quickly under a preheated warmer. Demonstrate your knowledge of Thermoregulation of the newborn by your documentation and role playing
Assign one minute Apgar score as you continue to assess and provide warmth to the baby.
Continue to stimulate baby  assessing respiratory and cardiac state, assign a 5 minute Apgar score
Clamp umbilical cord with a Hollister clamp, wrap baby and give to mother, skin to skin contact preferred, initiate breast feeding if feeding of choice

 Provide information  to family and significant others

 

 

 Staff will  be current with HIPAA regulations regarding communication with "Santa" and family. Demonstration of knowledge will be evaluated by use of documentation and role playing
Staff will maintain HIPAA standards and a  private environment
Staff will include patient wishes as well as HIPAA regulations in providing information to family members 
HIPAA guidelines will be maintained through out patient's stay in the emergency room.

 

Students will provide support to patient's and family as the delivery occurs and as completion of transfer arrangements are being finalized for mother and baby

 

 Information obtained from each group will be shared collectively. The staff will notify patient's Ob doctor as transfer begins to be organized 
Staff will communicate with receiving hospital pertinent information about the patient and her newborn baby. Implement these   learned tools in your role playing of the mock delivery. As you prepare mother and baby for transport and transfer  try some brain storming to obtain alternative    ideas in performing each others task.  As you complete all documentation for the transfer of mother and baby assure privacy of patients information by giving it out to only personnel who will be caring for the patient

 Completion of a plan of care by using the five steps of the Nursing Process

Using past  knowledge and collective data put into list form, use questions of who, how, where, and when to describe the events that could occur.  

Make a nursing diagnosis.  Interpret or predict the outcome by putting all the data together

Plan what steps to take and in what order and know why they need to be done. Implement your plans and make changes or improvements if needed by  evaluating the outcome. This is the final step in the nursing process.
