Case Scenario # 1

Annie Angina
Ms. Annie is an 70 year old women, 5'2' and 120lbs. She was brought into the emergency room by family members after she had been complaining about excruciating, chest pain that radiated to her jaw. Ms. Annie has a history of acute angina (Chest pain), hypertension and had been on nitroglycerin patch for chest pain. She admits to not adhering to her low sodium diet, enjoys eating out with her family and is unclear about her diet restrictions from past . After the cardiac work up was done it was revealed that Ms. Annie had an acute MI. her Vital signs were: HR- 130, RR- 35, T- 97.6 and BP- 195/100. Her hospital stay was short and she was ready for discharge. She will go home with prescriptions for Metoprolol 50 mg po BID, and Aspirin 325 mg po daily, Cardiac diet, and NTP patch 0.2 mg/hr to ACW daily.

Discharge Instruction Package
1). Using the same patient case scenario, the group will create an individualized discharge instructions package for your MI patient, who will soon be discharged from the hospital.
Each team member will contribute to this activity by providing individualized discharge/teaching information listed under your role, and according to your patient (case scenario), then coming together to create the package with all the information gathered.
