Case scenario # 4

Oliver Highfat

Oliver is a 40 year old male, 5'8" tall weighing 380lb and is experiencing shortness of breath and crushing chest pain radiating to the left arm. He enters into the emergency room and the  triage nurse take his vital signs: HR- 140, RR- 35, T- 98F, BP 200/110.  The nurse obtains a health history for Mr. Highfat: Diabetes, hypertension, diet- consumes a large amount of fast food daily because he has no time to prepare his own meals due to the great demand of his presence on his job.  Mr. Highfat does not participate in any exercise or fitness program he states that he watches movies on his DVD to relax and unwind.

Nursing Care Plan

1. After selecting a case scenario your team will come together to create a nursing care plan for that scenario, based on your prior nursing knowledge and the use of the nursing process.

Discharge Instruction Package

1). Using the same patient case scenario, the group will create an individualized discharge instructions package for your MI patient, who will soon be discharged from the hospital. 

Each team member will contribute to this activity by providing individualized discharge/teaching information listed under your role, and according to your patient (case scenario).  

Using the links provided select a  discharge instruction template for your case scenario so that you can organize, and write in your information.  Include patients  resources that they can use for further learning.  You will have 30 minutes to complete this section.
