Nursing Process

 

Assessment

In case studies they gather information based on the patient’s vital signs and symptoms, as well as diagnostic tests and procedures.  Here they identify problems related to the patient’s health status.

Diagnosis

Choose approved nursing diagnosis based on patient’s current or potential health problem which will be discussed in their presentation (i.e. knowledge deficit related to disease process).

Planning

Set goals on improving the quality of life for their patients, and uses them to formulate the nursing care plan and discharge instructions package (i.e. the patient will be able to verbalize appropriate care for his/her condition).

Implementation

Patient teaching will be implemented with the patients by the use of the discharge package.  All three roles come together and communicate regarding each patient’s individual interventions and goals.

Evaluation

The nurse will discuss the discharge packet with the patient allowing the patient to ask questions and comments which can give the nurse insight into the patient’s understanding.
