Activity # 1- Questionnaire

1.  First you'll be assigned to a team of 3 students, and each student in the team will select a role from the multidisciplinary health care team (Nurse, Dietician, Wellness Coordinator/Educator).

2.  Once you've decided on your role, you will answer the questionnaire (for your role) by using the links provided (resources) to find the required information.  You will have 30 minutes to complete the questionnaire.

Activity # 2-Nursing Care Plan & Discharge Instruction Package

Please choose one case scenario for your group and follow the directions.

Activity # 3-PowerPoint 

1.  All three roles in your team will come together to create a PowerPoint presentation using the information obtained from the care plan and the discharge instructions package.  You will have 60 minutes to complete this section. 

 2. Your group will present the discharge instructions using  PowerPoint  to the rest of the class.  Each of you will present the information that was gathered from your perspective (Nurse, Dietitian, or Wellness Coordinator/Educator).  You will have 15 minutes to present to your class.
