was designed in Microsoft Front Page and serves as a questionnaire.
Its basic function is for the teacher to get an idea of what the
students interests, homeroom teacher, grade and parent information
are. I used this survey to find out who would be interested in
joining a Rainforest club, and what they would hope to learn. All
of the information that is collected is sent back to the teacher in the
form of a database so it can be used at a later time.
